how to implement javascript in a php file

hi I am wandering if there is a possibility to implement a javascript into a php file and load it in html file. actually I would like to have a triplex combobox (see below the code) in a php file. when I run this script as a html, it loads without problem. but when I put it in a php file, it doesn't load.
any idea how should I implement it?
